SeniorCare Inc. is seeking a dedicated and compassionate individual to join our team as a Care Manager Administrative Assistant (CMA). This pivotal role not only supports our care management team by executing various administrative tasks crucial for our departmental efficiency but also serves as a key steppingstone for someone looking to advance into Care Manager (CM) position in the future.
The Care Manager Administrative Assistant (CMA) plays a vital role in our care management team, communicating with consumers, managing documentation, and providing essential administrative support. Responsibilities include communicating with consumers as directed by the care management team to confirm their continued need for services and explore additional support options. Additionally, the CMA assists care managers with check-in calls to consumers to confirm service satisfaction and assess ongoing needs and serves as backup for care manager on duty calls.
Document management is a key aspect of the role, maintaining accurate records of all communications with consumers, service providers, and other contacts. The CMA also monitors expiring care plans, reviews daily admission/discharge information to track consumer transitions and ensures closure of all appropriate cases monthly.
Qualifications for this position include a high school diploma or equivalent, exceptional communication skills, strong organizational abilities, and proficiency in computer applications. A compassionate attitude towards consumers and a commitment to confidentiality are essential.
