Advocacy & Navigating Care in the Home with Ongoing Risks Care Manager (ANCHOR)

SeniorCare Inc. has a full time ANCHOR Care Manager position which is part of an interdisciplinary team responsible for the provision of client-centered assessment, service planning and authorization provided to assist elders to live independently in the community. (ANCHOR) Advocacy & Navigating Care in the Home with Ongoing Risks is a highly focused care management and coordination level that provides a more frequent, rigorous and time intensive delivery of advocacy and other support to elders with behavioral health needs who are at risk of institutionalization or homelessness due to their inability to accept or retain services.

We are seeking an individual with Minimum of Bachelor’s degree in social work, human services, nursing or a related field. Commonwealth of Massachusetts Licensure in Social Work at the LCSW, LICSW or LSW level, or eligible, or licensure pending; or commitment to obtain said licensure within a reasonable time frame. Effective communication skills are necessary for interaction with the client, family, and other supports. Must be able to utilize a computer for data entry, word processing and to research resources. Experience with elders and/or in a community service setting is preferred.
